I have submitted a CRC to Stettler Minor Hockey within the last 3 years, and declare that no criminal charges or investigations have occurred or are ongoing since that time.

I would like to discuss matters in this regard privately with the SMHA Executive.

If selected as an SMHA coach this season, you consent to be registered under Hockey Canada and agree to their waiver agreements listed by clicking below.
